Names

Petitioner The person who filed the original case — the name at the top of the papers you were served with. Respondent The other party — this is usually you, if you were served. Other parent or party (optional) Most people leave this blank. Only fill it in if a third adult has a legal role in the case.

Court & case

Case number Look at the top of the papers you were served with. The case number is usually printed near the court's name. Superior Court of California, County of The county where the case is being handled. Court street address Court mailing address Often the same as the street address. Some courts use a separate mailing address. Court city and ZIP Department or room number (if known)

Hearing date Printed on the papers you were served. Your response is due at least 9 court days before this date. Hearing time Date of this declaration Today's date.
