Your full legal name (father / petitioner) Your legal name exactly as it appears on your ID or existing court orders. Other parent's full legal name (respondent) The other parent's full legal name — the name on their ID and existing court papers. County of filing The county where your existing case was filed. If you're opening a new case, this is usually the county where the children live. Case number Found on any prior order or paperwork from the court. If you don't have it handy, leave blank — the clerk can look it up by names. Court name Find this on your existing court order — e.g. "310th District Court." Leave blank if you don't know it yet. Date of the order you want to change The date the most recent custody or possession order was signed by the judge. It's on the front page of your order.

Your current legal designation under that order This is your official label in the court order. "Possessory conservator" is the most common for the non-primary parent. Not sure? Look at the section heading in your order that lists the parents.
